If your treatment is being funded by an Insurance company or your workplace, and you have one, please bring an authorisation form/number with you.
If you have had any other investigations such as X-rays or scans for this condition, please bring a copy of the report with you if you have it.
We would also welcome a referral letter from your GP or consultant if you have been given one, but these are not compulsory. You do not have to have been referred by another medical professional in order to come and see us.
